Some of us underwent extraction of tooth in our early life or we hear the experience of tooth extraction by our family members or relatives. The extraction of a tooth is fairly a quick process. It is the painless removal of the whole tooth under local anesthesia and pressure is felt by the patient. The extraction of a tooth is categorized into 2 types: - Simple extraction and surgical extraction. A simple extraction is mostly without complication and done by a dental surgeon. A surgical extraction is attaching with complication and there is a need for minor surgery in the oral cavity to reflect flap and remove bone from the periphery of the tooth and this case is done by an oral and maxillofacial surgeon


Indications for extraction: -

  • Tooth in the fracture line
  • Orthodontic purpose
  • Impacted tooth
  • Grossly decayed
  • Teeth directly involving cyst and tumor
  • Supernumerary tooth
  • Unrestorable tooth
  • Over retained deciduous tooth


A medical review before extraction: -

You must consult your physician about your medical condition before the extraction of the tooth. Share your medical history with the dentist to aware of the medicine you are taking right now or any previous history. This medical history helps to overcome the complication occur in between extraction, if you give proper medical history with the medications to the dentist. Dentists or physicians alter your medicine for a window period of extraction according to their need and after that period you again switch to the routine cycle of medication. Following are the conditions in which immediate extraction avoided: -

  • Uncontrolled diabetes mellitus
  • Hypertension
  • Bleeding disorders
  • Cardiovascular disorders
  • Liver disorders
  • Patient on long term steroid therapy
  • Teeth that have undergone radiation (6 month – 1 year)

Instructions after extraction: -

  1. Pressure pack for 30-45 minutes: -

It is kept at the extraction site for 30-45 minutes as this much time is needed to stabilize the fibrin.



    2.Avoid spitting 24 hours: -

It will avoid for 24 hours after extraction because it creates a negative pressure in the oral cavity and it may dislodge the clot and bleeding may occur.

    3.Avoid drinking from straw: -

Drinking with a straw can also lead to dislodging the clot and bleeding can occur due to negative pressure formation.

    4.Warm saline rinses: -

The warm saline rinses 4-5 times a day after 24 hours of extraction is very beneficial to the patient because the increased temperature of the fluid increases the blood flow to the healing site due to dilation. The increased blood flow increases the inflammatory cell at the site of extraction and enhances the healing process.

The addition of salt to the water makes the solution hypertonic this removes water/ fluid from the inflamed extraction side through osmosis therapy, reducing edema.

     5.Avoid taking hot food for 24 hours: -

Avoid taking hot food within 24 hours as it causes vasodilation and increases the risk of bleeding

     6.Drink something cold or eat soft and cold: -

After 1 hour of extraction, the pressure pack gauge/ cotton removes from the mouth and take something to eat or drink which is cold and soft (avoid straw and acidic beverages to drink). Cold temperature leads to vasoconstriction of the blood vessels and it reduces the chances of bleeding and helps in stabilization of the clot as well as act as a local anesthetic and decrease the swelling in the area.


      7.Avoid smoking after extraction: -

Smoking can lead to a negative pressure in the oral cavity as well as delay the healing process and leads to bleeding and it also contains nicotine which causes vasoconstriction leading to delayed healing and dislodgement of clot also.


         8.Avoid brushing at the site of extraction: -

Avoid brushing at the site of the extraction area because harsh brushing can disturb the healing process and bleeding can occur.

         9.Avoid the use of mouthwash: -

Avoid the use of very strong mouthwash, it may act as an irritant at the site of extraction.

         10.Avoid heavy exercise: -

Avoid heavy exercise for 1 to 2 days as it increases blood flow and may cause bleeding.

          11.Avoid playing instruments: -

Avoid playing instruments that are playing by mouth because it may create negative pressure and leads to dislodging of clot and bleeding can occur.


Professional Advice: - 

  • It's a myth that after the extraction of the tooth the eyesight going to weak. There is no harm to the eyes after the extraction of the tooth.
  • Provide proper medical and dental history to your dentist so that they can modify your treatment plan according to the need.
  • After extraction, if mild fever comes and goes at regular intervals, you can contact your doctor.
  • If you violate the instructions given by the doctor to you then, it may lead to a dry socket which is more painful.
  • If the swelling is increasing in the area after a day, then you can contact your doctor
  • Feeling pressure in the nearby area of the extraction site you can contact your doctor
  • If the pain is continuing more than 2 days after the extraction you can contact your doctor.
  • visit your dental clinic every 6 months for a regular check-up
